Tesla

UPDATE 23 June 2026: The article has been updated with Tesla's official statement and new information from the federal investigation.

Tesla with allegedly active FSD crashed into a Texas home a few days ago and killed one resident, adding to the growing list of fatal FSD-related crashes in the US.

Tesla Model 3 with active FSD crashed into a house, killing one

According to the report from the authorities, the driver behind a Tesla Model 3 was unable to keep a single lane, left the roadway and crashed into a house at high speed. A woman inside the house sustained serious injuries and was pronounced dead at the hospital. The driver was not intoxicated and was responsive during the investigation.

FSD-related incidents have gathered plenty of attention lately, forcing the NHTSA to initiate an investigation once again as the authority is looking into strange incidents involving Teslas ignoring stop lights and driving into the intersection or FSD performing a lane change into the oncoming traffic lane.

Tesla came out with an official statement saying that the Full Self-Driving system was indeed active, but the data also shows the driver pressing the accelerator to 100%, which overrode the FSD. The vehicle crashed at 73 mph with the accelerator floored, which remained at 100% even after the crash occurred. This leads us to believe that the FSD alone wasn't the problem.

This doesn't answer the question of why the driver pressed the accelerator in the first place. Sure, pedal misapplication is the reason the vehicle crashed, but what's the reason? The official federal investigation doesn't rule out FSD's involvement completely. FSD might have malfunctioned, and the driver might have pressed the wrong pedal in an attempt to intervene.

Obvious pedal confusion, usually by elderly drivers, which this one was not. My take is impairment of some kind. Was the driver tested?

" Full Self-Driving system was indeed active, but the data also shows the driver pressing the accelerator to 100%, which overrode the FSD" If FSD was over ridden then its not active during crash
